Please make sure to complete the necessary forms and return them to camp by June 1st unless otherwise noted.

Camper Health Form (to be filled out by the parent)
The information on this form is gathered to help us provide safe and appropriate health care. All information on this form is confidential and is kept in a separate locked file. It will be reviewed ONLY by the camp nurse, trip staff and/or Camp Administration. This form must be completed and signed by a parent or adult guardian of a camper under age 18.


Physical Form
This form requires a signature from the parent and physician for each camper every year. A physical exam must be done within 24 months before attending camp. The health form needs to be mailed or faxed prior to the session your child is attending (allow 3 weeks for delivery and processing).


Medication Form
Please list any necessary medications that your camper takes. This is for prescription medication as well as non-prescription medication. You must also list what you do not want your camper taking on this form.

Resident Camp PIL Sheet
This is for Resident Campers only. To help us better understand your camper’s needs, please complete our PIL and return it by May 15th. Use another sheet of paper if additional space is needed. Please assist your camper in filling in the camper section on the back. Use this time to have a conversation with your child before they come to camp. Your comments are very important in helping us to better serve your camper.
