Camper Forms

Forms Due by May 15th Log on to your CampInTouch Account. At the top right-hand corner of this page click on “Parent Login” or click here: CampInTouch Account Next click on the yellow icon with the pencil for access to all of the necessary forms.           These forms you will complete online: Health History Form (to be filled out by the parent) & Immunizations  This form must be completed and  electronically signed by a parent or adult guardian of a camper under age 18. If you register after May 15th please have forms in 3 weeks prior to the session your child is attending. Behavior Agreement (to be filled out by both parent and camper) Please take a few moments to review the Camper Behavior Agreement with your camper. Please signify you both understand and agree to each statement by checking each of the boxes to the left. Parents and campers must complete this form before coming to camp. Use this time to have a conversation with your child before they come to camp. Parent Information Letter (to be filled out by parent) This form helps us to better understand your camper’s needs. Your comments are very important in helping us to better serve your camper. These forms needed to printed: Once they are completed please scan and upload to your CampInTouch Account : ***MUST BE PDF FORMAT   Physical Form (to be filled out by doctor or nurse) this form requires a signature from the physician every year. A physical exam must be done within 24 months before attending camp. Winter Camp

SIGN UP FOR WINTER CAMP! Come back to camp! Phantom Lake’s Winter Camp is four days full of exciting camp activities! The only difference is- it’s in the snow! Winter camp runs from the 27th of December between 10 and 11 a.m. until December 30th between 10:30 and 11 a.m. and is open to any and all 7-15 year olds! Join us this winter for sledding, outdoor activities, and crafts! As well as gingerbread bread house making, board games, cards, fort building, a camper & staff show and so much more! Who knows…there might even be a crier! Campers bunk in the bottom half of Erich Lodge also known as the Dining Hall, the sides are divided up by ladies and gentlemen. It is fully heated for a comfortable night’s sleep. Hot coco is readily available and each meal will be served family style in each camper’s group. Winter camp is great time to come see camp when you normally would not, while kicking back and relaxing in front of the fire with some of your favorite counselors from the summer. Come join in the fun, make new friends from all over the place or reunite with old friends. Get ready to learn new games and play some camp classics!
